These instructions apply to all policies and contracts of coverage except for coal mining policies.&lt;/p&gt;
&lt;p&gt;Every policy requires a first report of the premium and losses valued as of eighteen months after the policy effective date. The first reports are submitted to the MWCIA between or during the 18th through 20th month after the policy effective month.&lt;/p&gt;
&lt;p&gt;The losses are valued and reported at 12 month intervals following the first valuation and report.
